เงื่อนไขการรับบริการ
1. ขอสงวนสิทธิ์การให้บริการสำหรับท่านที่นำ E-Coupon มาแสดงเท่านั้น
2. E-Coupon นี้ไม่สามารถแลกเปลี่ยนเป็นเงินสดได้
3. E-Coupon นี้สำหรับการตรวจตามรายการที่ระบุเท่านั้น ไม่สามารถเปลี่ยนแปลงได้
4. สามารถใช้สำหรับผู้รับบริการผู้ป่วยนอก (OPD) และผู้รับบริการที่ช่วยเหลือตัวเองได้เท่านั้น
5. ชุดตรวจสุขภาพ ไม่รวม ค่าแพทย์ในกรณีเจาะจงแพทย์ผู้เชี่ยวชาญในการตรวจสุขภาพและตรวจทางรังสี
6. ชุดตรวจสุขภาพ ไม่รวม ค่ารักษาหรือค่ายาเพิ่มเติมในกรณีที่แพทย์สั่ง
7. กรุณานัดหมายล่วงหน้า และงดอาหาร ก่อนเข้ารับบริการ 8-10 ชั่วโมง (จิบน้ำเปล่าได้)

9. สามารถรับบริการที่ เฮลท์ ดีไซน์ เซ็นเตอร์ ชั้น 5 อาคาร R โรงพยาบาลกรุงเทพ (ซ.ศูนย์วิจัย) ทุกวัน 7:00 – 15:00 น.

วันหมดอายุ E-Coupon 
1. E-Coupon มีอายุการใช้งานภายใน 180 วันนับจากวันที่สั่งซื้อ
2. หาก E-Coupon หมดอายุ รพ.ขอสงวนสิทธิ์การเข้าใช้บริการ และ E-Coupon ไม่สามารถขยายระยะเวลาในการใช้บริการหรือคืนเป็นเงินได้
3. แพ็กเกจสุขภาพราคานี้ สามารถสั่งซื้อได้ถึง 31 มกราคม พ.ศ. 2569 เท่านั้น

หมายเหตุ
1. ในกรณีที่ลูกค้าประสงค์จะยกเลิกคำสั่งซื้อด้วยเหตุผลที่ไม่เกี่ยวข้องกับโรงพยาบาลหรือเหตุผลทางการแพทย์ โรงพยาบาลขอสงวนสิทธิ์ในการคืนเงินหรือยกเลิกสินค้า
2. ยอดเงินที่ระบุในใบเสร็จเป็นยอดชำระจริง ไม่รวมส่วนลดจากโปรโมชั่นใด ๆ
3. หลังการสั่งซื้อ ลูกค้าจะได้รับใบเสร็จในรูปแบบออนไลน์เท่านั้น หากต้องการใบเสร็จตัวจริงในวันที่เข้ามารับบริการ สามารถแจ้งขอได้ที่เจ้าหน้าที่การเงิน

1. Service is reserved exclusively for customers who present the E-Coupon.
2. This E-Coupon cannot be exchanged for cash.
3. E-Coupon is only applied to specific items as mentioned and changes cannot be made.
4. These packages are for outpatients who are physically dependent. 
5. These packages exclude doctor fees in case additional specialists are required.
6. These packages exclude treatment fees or medications prescribed by the doctors, if any. 
7. Please make an appointment in advance and refrain from eating 8-10. hours before the service  (drinking water is allowed).For further information or to make an appointment, please contact the Health Design Center at 02-3103284, 02-7551591 (operating hours: 07:00-15:00) or the Contact Center at 1719.
8. Services can be received at the Health Design Center, 5th Floor, Building R, Bangkok Hospital (Soi Soonvijai), 
every day from 7:00 to 15:00.

Expiration Date of E-Coupon:
1. The E-Coupon is valid for 180 days from the date of purchase.
2. If the E-Coupon expires, the hospital reserves the right to refuse service, and the E-Coupon cannot be extended or refunded.
3. This health package can be ordered until January 31, 2026.

Note:
1. In cases that customers request to cancel an order for reasons not related to the hospital or medical purposes, the hospital reserves the right not to issue a refund or cancel the product at its discretion.
2. The receipt amount displayed is the actual payment received, excluding any promotional discount.
3. After completing the purchase, customers will receive an online receipt only. If a physical receipt is required on the day of the visit, it can be requested from the cashier.
